Animal Oasis
A non-profit organization devoted to the welfare of animals in Box Elder County

We want to make sure each adoptive parent provides a stable and loving environment for his or her new companion pet. Because of this desire, our adoption application is very through. You must be 18 years old or older to adopt from Animal Oasis.

Dog Adoption Application

Note: The Animal Oasis or participating foster homes as well as any professional pet boarding kennels are all separate entities. While the dog you are interested in may have been housed and cared for at a professional pet boarding business or in a foster home, they are not an animal shelter and are not the same as the Animal Oasis.

Do you agree to contact The Animal Oasis if, for any reason, you can no longer keep this dog? Understanding this notification does not necessarily mean you must surrender the dog, and that the pet boarding business or applicable foster home is not liable to, and may not be able, to accept this pet back at any time. Yes No

All the above information I have given is true and complete. Should an un-spayed or un-neutered dog be placed with me, I agree to have it spayed or neutered no later than 30days, and notify the Animal Oasis. The dog will reside in my home as a pet. It is solely my responsibility to provide adequate food, water, shelter, training, affection, safety and medical care. I am in full agreement with the Terms of Adoption. The Animal Oasis, professional pet boarding business or applicable foster home is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog in my household. Yes No

Animal Oasis does not charge for the adoption of a dog, other than to recover expenses. Operating a non-profit animal rescue organization in a caring and professional manner is important to the members of Animal Oasis, and can only be done through donations. No one involved in this organization has received any profit

A donation is requested for the dog you are interested in. Understanding that the requested donation is just that, and should you return this dog you will not receive your donation back.

The above statements and your answers constitute a binding contract. We have and will prosecute any one in breach of our contract. We are available long after the adoption of your animal for questions and to address your concerns. We expect our adoptive families to follow through on their commitments to this adoption. We expect our adoptive homes to hold true to the above statements in this application and those in the adoption agreement form, which will be signed before the adoption is finalized. This commitment includes providing a proper environment cleaned regularly, food, water and veterinarian care. It also includes returning the pet to us in Brigham City should the adoption not work out. We will not trade or "swap" animals once an adoption is made. Our contracts are binding and have stood up in court! The fines of $500 and court costs will be enforced for any one found to be in violation of our contract. We take very seriously our responsibility to these animals and expect no less from you. Animals are adopted on first come first served basis. We will not hold any animal.

We reserve the right to refuse adoption to anyone, for any reason, at any time, in accordance with our own common sense, experience and sense of responsibility and love for the animals that we rescue.
