Animal Oasis
A nonprofit organization devoted to the welfare of animals in Box Elder County

We want to make sure each adoptive parent provides a stable and loving environment for his or her new companion pet. Because of this desire, our adoption application is very through. You must be 18 years old or older to adopt from Animal Oasis.

Reptile/Exotic pet Adoption Application
Note: The Animal Oasis or participating foster homes as well as any professional pet boarding kennels are all separate entities. While the reptile you are interested in may have been housed and cared for at a professional pet boarding business or in a foster home, they are not an animal shelter and are not the same as the Animal Oasis.

Do you agree to contact The Animal Oasis if, for any reason, you can no longer keep this reptile? Understanding this notification does not necessarily mean you must surrender the reptile, and that the pet boarding business or applicable foster home is not liable to, and may not be able, to accept this pet back at any time. Yes No

All the above information I have given is true and complete. Should an un-spayed or un-neutered reptile be placed with me, I agree to have it spayed or neutered no later than 30days, and notify the Animal Oasis. The dog will reside in my home as a pet. It is solely my responsibility to provide adequate food, water, shelter, training, affection, safety and medical care. I am in full agreement with the Terms of Adoption. The Animal Oasis, professional pet boarding business or applicable foster home is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog in my household. Yes No

No one involved in this organization has received any profit

A donation is requested for the pet you are interested in. Understanding that the requested donation is just that, and should you return this reptile you will not receive your donation back.
